When a care worker completes a falls risk assessment, the relevant information maps automatically to the right sections of the resident's care plan. When that assessment is reviewed and updated, the care plan updates with it. 

This means your care plans are always current, always evidence-based, and always person-centred, without anyone having to maintain them manually.

Automated medication management (eMAR)

Access Point of Care addresses this directly with a fully integrated electronic medication administration record (eMAR) that connects medication management to the rest of the care record. 

Barcode scanning acts as a secondary check at the point of administration, confirming the right medication for the right resident at the right time, using the camera on the device already in the care worker's hand.

The app is built around how a care worker's day actually works 

On login, staff select their location and are immediately presented with their activities for the shift: what is overdue, what is due now, and what is coming up. This covers both care activities and medication rounds in a single view so that nothing is missed because it was in a different system.

The resident profile is designed to look and feel like a paper folder, but with the depth and searchability that paper can never provide. 

From a single profile, staff can see: 

  • A full timeline of everything that has happened during the resident's time in your care, including both care and medication records 
  • Active alerts and any action plans in place to address them 
  • Outstanding actions and scheduled activities 
  • Any custom indicators configured for that resident 

The timeline can be filtered by type - medications, treatments, charts, care observations, or custom actions - so staff can find exactly what they need quickly.

No two care homes are identical

Access Point of Care includes a library of standard forms and charts covering the assessments and documentation your team needs day to day, but every form and chart can also be built or adapted to fit your organisation's specific requirements. 

The forms and charts builder uses a flexible, survey-style approach that supports conditionality (questions that only appear when relevant), images, icons, video guidance, and custom terminology. If a resident is low risk, the system moves on. If a risk is identified, the system asks the right follow-up questions to capture it properly. 

Custom charts work the same way, built around your care processes, your specialisms (whether that is dementia, palliative care, learning disabilities, or supported living), and your documentation standards.

Intelligent alerts and escalations 

Access Point of Care monitors what is happening across your residents and your operations, and surfaces the things that need attention rather than leaving managers to go looking for them. 

The home screen dashboard gives a real-time RAG (red, amber, green) status across key areas: active alerts, missed actions, medication round progress, and exception codes. Clicking through from any item takes you directly to the relevant resident or record. 

Escalation pathways from chart inputs are also available, allowing care staff to raise concerns directly within a chart and notify a manager for prompt action, reducing response times and strengthening accountability. 

95% pharmacy connectivity 

Access Point of Care connects directly with 95% of UK pharmacies, making medication ordering and stock management significantly simpler. Rather than managing pharmacy relationships manually, the system handles the communication, reducing the administrative overhead and the risk of running out of critical medications. 

Secure access 

Access Point of Care uses Access Identity for secure login, with multi-factor authentication (MFA) available.  

Full offline capability on mobile 

Connectivity in care homes is not always reliable. Access Point of Care works fully offline - if a care worker moves into an area without a signal, the app switches to offline mode automatically, caches everything entered, and syncs as soon as the connection is restored.
